Important Information for Donors
- The Oregon Dept. of Revenue requires that we collect donor Social Security Numbers or Employee Identification Numbers.
- There is a $500,000 limit on tax credits an individual can claim on contributions per return filed. That would be a contribution of $555,555.
- The maximum tax credit rate for a credit card donation is 90%.

Step 2: Send Your Donor Form to Neighborhood Partnerships
If you have donated before, you may send your form without your social security number to email@example.com. If you have not donated before, please include your SSN and follow the mailing instructions below. For security purposes, please do not email your form to us if it contains a social security number.
